REQUESTING DISABILITY SUPPORT RECORDS:

Former students and graduates of Lenape Valley Regional High School who would like to have disability support records issued to colleges should complete the attached form and submit it to the Special Services Department.  These records might include Individual Education Plans (IEP), Senior Exit Testing Evaluations, etc.  More information about these records and anything else pertaining to the Special Services Department can be found on the Lenape Valley Regional High School Special Services Department website:

REQUESTING IMMUNIZATION AND HEALTH RECORDS:

For any former student who is requesting Immunization or Health Records please click on this online Google Form and our Guidance Department will process your request.

REQUESTING TRANSCRIPTS:

Lenape Valley High School Former Students and Graduates of more than one year:

Transcript requests and employment verifications for former LVRHS students are now managed through an external service, Parchment.

Cost:

$5.00 per transcript for alumni 

$20.00 per Education Verification

 

Directions:

  1. Visit  https://www.parchment.com/u/registration/17133/account

  2. Create a new account, or use an existing Parchment account (if you already have one). Former students should create a new learner account.

  3. Fill in the requested information to create the account.

  4. After verifying your account, click the green "Order" button, next to transcript.

  5. You will need to know where you need to send the transcript. You can search for most colleges/universities, but will need to enter information for most other recipients. Be sure to have this information ready before you begin. You are responsible for selecting the correct location.

  6. Electronically sign to give consent, and review your order. You can request multiple transcripts at once, and pay at the end.

  7. Complete the online payment process.

You can track your order at any time by returning to your account, logging in, and clicking the Orders tab.

 

Lenape Valley High School Recent Graduates of less than one year:

For alumni who have graduated less than one calendar year ago, there is no cost to fulfill your transcript request.  You will complete this online google form and our Guidance Department will send your transcript where it needs to go.
